Medicare Annual Wellness Visit Patient Overview
NHN offers two resources to help educate your patients on the importance of a Medicare Annual Wellness Visit. Choose from two different versions, one of which includes a patient planning section to help beneficiaries prepare for the visit. Both versions are available in English and Spanish.
Medicare Annual Wellness Visit Patient Poster
This 12 x 18" poster can be displayed throughout your clinic to help educate patients about the benefits of Medicare Annual Wellness Visits. The poster features a side-by-side comparison between an annual exam and an AWV and highlights aspects of the exam that may be billable. Contact the NHN if you would like paper copies delivered to your clinic.

Medicare Annual Wellness Visit Education Module
The Medicare Annual Wellness Visit connects patients and providers to review health records, discuss preventive health-care services, such as screenings and vaccinations, and to develop an overall care plan. The education module is approved for 0.25 CMEs from the American Medical Association.
