With representation from the two leading health systems in our community, you always have access to the best care possible.

Improved Care Coordination

Part of what makes an ACO different is the enhanced focus on your preventive health needs and the collaboration across your entire care team. NHN’s clinical coordinators work hand-in-hand with your provider to proactively identify appointments, screenings and procedures most appropriate for you. This approach ensures that all patients receive the right care, at the right time.

Lower Health-Care Costs

NHN works with insurance providers, Medicare and Medicaid to develop value-based contracts that govern our care. Value-based insurance contracts set specific cost and quality standards that are expected of our providers. This added level of oversight leads to better patient outcomes and lowers the overall cost of care by preventing unnecessary or duplicative services, minimizing your risk of hospitalization and reducing the likelihood of ER visits.
